We are an Alabama Coordinated Health Network (ACHN) providing care coordination services in Primary Care, Maternity, and Family Planning to Medicaid Recipients. Our Care Coordination team includes nurses, social workers, pharmacists, and community health workers. Your Care Coordinator is trained to work with your doctor to help you make the best choices to get healthy and stay healthy.

We serve Alabama Medicaid Recipients in Cullman, Jackson, Limestone, Madison, Marshall, and Morgan Counties.
